Residential Services
- In a Host Home, individuals live with one of Treasured Circle’s host home providers in a safe, stable and nurturing environment, and receive daily support tailored to meet their needs from the host home provider and from Treasured Circle. Each placement is thoughtfully designed around the individual’s needs, preferences, values, and goals.
- Personal Care Alternative allows an individual to lease their own apartment and receive services on an as-needed basis. Services received can be for up to 24 hours a day, seven days a week, depending on the needs of the individual
being supported.

Treasured Circle maintains close communication and oversight with all host home providers to ensure consistent quality, compliance, and individualized care. We ensure that our placements are where individuals are valued, supported, respected, and empowered to grow. We are confident that individuals referred to our agency will receive the supportive and nurturing environment needed to thrive.
Treasured Circle is proud to offer a network of trained and experienced host home providers who create supportive, safe, and family-centered environments for individuals who reside or would like to reside in host homes. With the consistent support from Treasured Circle, our host home providers help the individuals they support to thrive and continuously ensure the highest quality of care is delivered in every home.
At Treasured Circle, our host homes provide:
- A safe, clean, stable and welcoming living environment, with 24/7 support from trained and compassionate host providers.
- Assistance with daily living skills; support with personal goals and life skills.
- A person-centered atmosphere where each individual’s preferences, strengths, and goals guide daily support.
- A balanced structure that encourages independence while offering reliable guidance and assistance as needed.
- A strong focus on health and wellness, including medication support, appointment coordination, healthy habits, and ongoing monitoring of well-being.
- Community inclusion opportunities, helping individuals engage socially, develop skills, participate in activities, and build meaningful connections.
- Respect, dignity, and privacy, ensuring individuals feel valued, understood, and empowered.
- Skilled, compassionate providers, trained in essential caregiving practices such as CPR/First Aid, medication administration, behavioral support strategies, and person-centered planning.
- Ongoing monitoring, training, and support from our agency team.
Services Offered
Personal Care Alternative (PCA)
Personal Care Alternative allows an individual to lease their own apartment and receive services on an as-needed basis. Services can be received 24 hours a day, seven days a week, depending on the individual’s needs.
Services Offered
Respite Services
This service is typically available on a short-term basis due to the absence or need for relief of those individuals normally providing care. It is mainly provided at your home or at a provider’s home, and options include hourly supervision and/or daily supervision.
Hourly supervision is usually under 10 hours, while daily supervision is 10 hours or more per day, up to 24 hours.

Personal Care
This service is designed to support individuals with daily activities they would typically perform independently but may need assistance with due to physical, medical, or functional limitations. These services promote comfort, dignity, safety, and independence in everyday life.
- Bathing and personal hygiene
- Skin, nail, hair, and oral care
- Shaving, dressing, and grooming
- Eating and meal-related support
- Walking, exercise, and mobility assistance
- Transferring and positioning
- Toileting and continence care
- Accompaniment for grocery shopping
- Support during medical or community appointments related to personal care needs
- Protective Oversight, providing supervision to ensure safety for individuals with specific monitoring needs
Services Offered
Homemaker Services
Homemaker Services provide training and hands-on assistance with essential household tasks to help individuals (those who cannot perform this service independently) maintain a safe, clean, and comfortable living environment. These services are designed to support safety, comfort and independence, offer guidance and build skills, while ensuring daily household needs are met.
Homemaker Services May Include:
- Meal planning and cooking
- Laundry and clothing care
- Cleaning and organization
- Assistance with routine household maintenance tasks
- Shopping and appointment management
Services Offered
Mentorship Services
Our Mentorship Services are designed to promote self-advocacy, confidence, and informed decision-making. Through individualized guidance, modeling, advising, instructing and providing experiences, individuals are supported in developing the skills needed to become more independent and actively participate in their communities.
Mentorship Services May Include:
- Instruction and guidance in self-advocacy skills
- Learning through shared experiences and real-life practice
- Modeling effective communication and decision-making
- Advising and support tailored to individual goals
Mentorship Services Can Help Individuals:
- Interview and select potential service providers
- Understand complex health, safety, and service-related information
- Participate in private and public boards and advisory groups
- Build leadership skills and community involvement
How We Provide Support:
Mentorship Services are provided through consistent, one-on-one support that is tailored to each individual’s goals and strengths. Mentors build trusting relationships and create supportive environments where individuals feel comfortable to ask questions, express their needs and concerns, and practice self-advocacy skills.
Our mentors help break down complex information into clear, understandable steps, empowering individuals to make informed choices. Support is provided in a way that builds confidence, promotes independence, and respects each person’s voice, preferences, and rights. Mentorship focuses on skill-building, gradual independence and long-term success, so individuals are better prepared to advocate for themselves in both personal and community settings.

Supported Community Connections
Our Supported Community Connections services help individuals access and participate in the typical activities and functions of everyday community life. These services are designed to promote inclusion, independence, and meaningful engagement within the community.
Supported Community Connections May Include:
- Participation in community outings, education and training opportunities
- Volunteering and civic involvement
- Learning to use public transportation safely and independently
- Planning for and attending community events and activities
- Building and maintaining positive social relationships
How We Support:
Support is provided in a person-centered manner that focuses on individual interests, strengths, and goals. Staff work alongside participants to encourage skill development, confidence, and independence while ensuring safety and accessibility. Emphasis is placed on real-world experiences, community integration, and building natural supports that help individuals feel connected and valued within their community.
Services Offered
Life Skills Training
Life Skills Training is an individualized service designed to help individuals develop, strengthen, and maintain the skills needed to promote independence, confidence and stability in everyday life. Life Skills Training plays a vital role in helping individuals build the knowledge, confidence, and abilities needed to live successfully and independently in the community. By focusing on practical, everyday skills, this service supports individuals during times of transition or change, helping them adapt to new environments and responsibilities. Treasured Circle plans and directly collaborates with you, to provides service in your residence, within the community, or in a group living setting. As skills are developed, support is gradually adjusted to encourage greater independence, helping individuals build the tools they need for long-term success.
You may be eligible for Life Skills Training if you are:
- Transitioning from an institutional setting to a home and community-based setting
- Transitioning from a more restrictive community setting to a less restrictive community setting
- Experiencing a significant change in life circumstances that impacts your ability to live independently
Life Skills Training may be provided for up to 365 days following the first day the service begins, based on individual needs and service authorization.
Life Skills Training may include instruction and hands-on practice in areas such as:
- Daily living routines and personal organization
- Meal planning, grocery shopping, and basic cooking skills
- Household management, cleaning, and laundry
- Budgeting, money management, and paying bills
- Time management and scheduling
- Using community resources and services
- Transportation planning and travel training
- Communication, problem-solving, and decision-making skills
- Health, safety, and wellness awareness
Life Skills Training promotes independence by teaching skills that are essential for daily living, decision-making, and problem-solving. It empowers individuals to take an active role in their own lives, make informed choices, and develop routines that support stability and well-being. Beyond skill development, Life Skills Training helps reduce reliance on more restrictive supports by fostering self-sufficiency through hands-on practices, guidance, repetition, and positive reinforcement. It encourages personal growth, community participation, and long-term success by providing individuals with the tools they need to maintain their independence, safety, and quality of life.

To be considered for Host Home Provider, Treasured Circle
requires that applicants meet these qualifications:
Personal Requirements
✓ Be at least 18 years old with high school diploma or equivalent
✓ Be honest, dependable and caring with good judgment
✓ Have strong communication and organizational skills
✓ Be patient and understanding
Home Requirements
Your home (owned or rented) must provide:
✓ A private bedroom for the individual (bathroom can be shared)
✓ Common living areas (kitchen, living room, dining room)
✓ An outside yard for shared use
✓ Safe, clean environment
Background & Clearance
✓ Successfully clear criminal background check
✓ Good driving record and reference checks
✓ Valid driver’s license (or family member available)
✓ All household members over 18 must clear background checks
✓ Eligible to work in developmental disability system
Training & Certification
Required certifications (we provide training):
✓ QMAP (Qualified Medication Administration Personnel)
✓ First Aid
✓ CPR
✓ Legal Rights training
✓ Mistreatment, Abuse and Neglect reporting
Insurance & Equipment
✓ Full automobile insurance (meeting state minimums)
✓ Home/renter’s insurance (recommended)
✓ Computer with internet access
✓ Required documentation equipment
